The Department of Nuclear Medicine at Safdarjung hospital is one of the oldest in the country.
Nuclear Medicine Department is the only hospital under Central Health Services to have this advanced facility in Delhi NCR after AIIMS, New Delhi.. It offers state-of-the-art diagnostic facilities to indoor patients and outdoor patients.

The department is located in the Zone-1, Basement -1, SSB Block and Ground Floor and also in H- Block of VMMC & Safdarjung Hospital.

First department of Nuclear Medicine in India was established at Safdarjung Hospital 50 years ago. After AIIMS, New Delhi, VMMC and Safdarjung Hospital is the only hospital under Central health services to have this advanced facility in Delhi NCR.
Nuclear Medicine:
Nuclear medicine is a branch of medicine that uses small amounts of radioactive material (unsealed sources of radiation) to diagnose or treat a variety of diseases, including many types of cancers, heart disease and certain other abnormalities within the body. Organ /tissue specific compounds, known as radiopharmaceuticals, are administered to the patient for diagnostic and therapeutic procedures. Nuclear medicine imaging and non-imaging procedures provide important information about functional status of the body organs. Nuclear medicine or radionuclide imaging procedures are noninvasive and usually painless medical tests that help physicians diagnose medical conditions.
Radiopharmaceuticals are administered to the patient for diagnostic and therapeutic procedures. Nuclear medicine imaging and non-imaging procedures provide important information about functional status of the body organs.
Radiotracer technology has made it possible to define disease in terms of physiology and biochemistry rather than anatomy or histopathology. Whereas Ultrasound, computed tomography (CT) and magnetic resonance imaging (MRI) define disease on the basis of abnormal structure, Positron Emission Tomography (PET) and Single Photon Emission Tomography (SPECT) imaging permits 3D reconstruction of data, increasing the sensitivity and anatomical localization of lesions in the skeleton, brain and heart. The sensitivity of localization of tumors is also increased. In Nuclear Medicine the emphasis is on function and chemistry rather than structure.
Radionuclides are also used for therapy of malignant and non-malignant conditions. A lot of progress has taken place over the past few years in Therapeutic Nuclear Medicine. With the use of suitable radiopharmaceuticals targeted therapy is also possible. Nuclear medicine uses small amounts of radioactive material to diagnose or treat a variety of diseases, including many types of cancers, heart disease and certain other abnormalities within the body.
Non-Imaging procedures e.g. Thyroid Uptake Tests are also being done.

BRIEF ABOUT PET-CT
A new state-of-the-art PET-CT system (Siemens Biograph mCT 64-slice) worth ₹20.6,00,00,00 Cr.has been installed in the SSB block department of Nuclear Medicine.
PET-CT has become an essential tool in the diagnosis, prognosis, treatment planning, and follow-up of cancer patients. Following PET-CT Scans are being carried out in the Department.

It has particular clinical relevance in the management of:
Hodgkin’s lymphoma and Non-Hodgkin’s lymphoma
Lung cancer, Neuroendocrine Tumors (NETs), and Prostate cancer
Advanced PET imaging studies including FDG-PET, DOTANOC, FAPI, PSMA, and other specialized scans.
Myocardial Viability
The facility also supports multiple clinical departments, including Medical Oncology, Radiation Oncology, Hematology, Urology, Nephrology, and Pediatric Oncology.
PET-CT imaging is essential prior to the administration of radionuclide therapies such as Lu-PSMA and Lu-DOTANOC for cancer patients, ensuring accurate patient selection and optimal treatment planning.
Beyond oncology, PET-CT also plays an important role in the evaluation of complex and difficult-to-diagnose conditions in cardiology and neurology, further strengthening the hospital’s comprehensive patient care services.
BRIEF ABOUT DUAL HEAD GAMMA CAMERA (SPECT-CT)
A new state-of-the-art Dual head Gamma Camera ( AnyScan SPECT-CT System with 16 slice CT Scan) worth ₹7.89 Cr. has been installed in the SSB Block, Department of Nuclear Medicine. After AIIMS, New Delhi, VMMC and Safdarjung Hospital is the only hospital under Central health services to have this advanced facility in Delhi NCR.
SPECT-CT has become an essential tool in the diagnosis of:
Gastrointestinal system
HIDA Scan: to diagnose bile leak, extrahepatic biliary atresia, gall bladder ejection fraction.
Tc99m Pertechnetate scan: To diagnose ectopic gastric mucosa eg. Meckel’s.
GI Bleed Study: to diagnose the site of intermittent G.I. Bleed.
Salivary gland Scintigraphy: to diagnose obstruction to the flow.
Genitourinary system
DTPA Scan: to diagnose the GFR of kidneys especially differential GFR & drainage from the PCS. Level of obstruction in the urinary tract.
DMSA Scan: to diagnose renal cortical defect and acute / chronic pyelonephritis. Locate Ectopic kidney.
DRCG study: to diagnose vesicoureteric reflux.
Testicular scan: to diagnose testicular torsion.
Skeletal System
MDP bone scan (whole body): to diagnose skeletal metastasis.
MDP bone scan (three phase): to diagnose osteomylitis and loosening vs infection of prosthesis.
Endocrinology:
Thyroid scan: Hot / cold nodule in the thyroid gland.
Parathyroid scan: to diagnose parathyroid adenoma and determine its location.
Cardiology:
Myocardial perfusion scan: to diagnose stress induced reversible perfusion defect. Left ventricular wall motion abnormalities.
M.U.G.A. scan: it is gold standard test to diagnose Left Ventricular ejection fraction.
V/Q scan: to diagnose pulmonary thromboembolism.
Brain Scan (with Technitium -GHA / ECD / HMPAO): to diagnose abnormalities in cerebral perfusion.
NON-IMAGING DIAGNOSTIC FACILITIES:
- BRIEF ABOUT THYROID UPTAKE PROBE WITH WELL COUNTER
This equipment Thyroid Uptake System with a well counter is used in following cases
In patients suffering from thyroid disorders-helping in differentiating Grave’s disease from various Thyroiditis.
In planning 131-Iodine therapy for Grave’s disease patients.
In planning high-dose 131-iodine therapy for Thyroid Cancer patients.
Thyroid uptake probe with well counter is available for in-vivo and in-vitro studies.
THERAPY:
Department has facility to provide radionuclide therapy facility to patients of thyrotoxicosis. Other radionuclide therapy include bone pain palliation with 32P,
131-Iodine to patients of thyrotoxicosis.
Department is in advance stage of starting the following therapies:
Lu-PSMA Therapy for Cancer Prostrate
Lu-DOTANOC Therapy for NET
FUTURE PLAN OF ACTION:
1. The tendering process for procurement of a 2nd PET-CT and 2nd SPECT-CT (Gamma Camera) equipment is currently in progress.
2. Establishment of a Nuclear Medicine High-Dose Therapy Isolation Ward for patients
with Thyroid Cancer, Neuroendocrine Tumors (NETs), Prostate Cancer, and other
Oncological conditions is under progress.
